Here at Singular Sleep, we strive to make your journey to sleep health as easy and painless as possible. In order to stick to those values we have provided a variety of insurance claim forms for you. Please choose which insurance provider you have, and print out the attached form. Fill it out in it's entirety and mail it to your insurance provider with the invoices provided to you by Singular Sleep. We have pre-filled parts of the provider section for your convenience.
