 Program Terms and Conditions: 

• To be eligible to receive the $25 Amazon gift card, the referrer party must have already purchased a home sleep study through Singular Sleep, returned the testing equipment, and received his/her test results. 
• The referred customer will receive 10% off his or her sleep test fee.
• The referred customer must reside in one of the states where our physician is licensed to read sleep studies. 
• Customers who requested a refund (partial or full) for a home sleep study are excluded from the program. 
• There is no limit on referrals.
• The referring customer's name must be listed in the "Referred By" section of the home sleep study order form at the time of the form's electronic submission or on the Referral Form.
• We will send the $25 Amazon gift card electronically to the referring customer's email address that we have on file. 
• This offer cannot be combined with any other home sleep study special offers or promotions.
